Customize Your Feedback Form

Use headers, themes, colors, and fonts to make your form visually appealing.

In this video, you will choose a theme, color scheme, image, and font that illustrate your feedback form’s content.

Make your feedback form stand out by choosing images and colors that fit the theme of your project.

Use design elements that make your survey look more professional, or lighthearted.

Or, just choose ones that look great.

To begin, open the feedback form you created in the lesson.

Select a different header from the options shown.

Look over some of the headers provided in Forms and choose one that makes sense for your feedback form.

For example, if you are looking for feedback on a science project, you can choose a science-themed header.

Or, just pick a design that you like.

You can also upload a photo to use as a header from your own device or Google Drive.

Once you have chosen your header, select a theme color for the rest of your form.

You can choose one of the available options, or add a new color.

Change the background color to enhance the look of your feedback form.

Finally, get the font style to make your form interesting and easy to read.

Now, it’s your turn: Select a new header, choose a color theme and background color, and change the font on your form.


Adjust Form Settings

Revise and finalize your questions and select settings, including anonymity.

In this extension, you will adjust the settings on your feedback form.

These settings allow you to make choices about the information you give to and receive from your feedback partners.

They also allow you to decide how your feedback form will be presented.

Choose settings based on the type of form you want to create and information you’re looking to collect.

To begin, open the form settings.

You have the option to collect email addresses from your feedback partners.

If you collect email addresses, you will be able to see which partner gave which answer.

If you do not collect email addresses, your feedback partners can fill out your form “anonymously,” without leaving their name.

This is especially helpful for surveys of a personal nature, or when you are inviting people to give tough or sensitive feedback.

Collect email addresses for this feedback form.

Next, choose whether to send your feedback partners a copy of their answers.

Send them a “response receipt” so they know their answers have been recorded.

Limit your feedback partners to one response.

Once they submit the form, they will not be able to resubmit.

Finally, decide whether you want your feedback partners to be able to edit their answers after they send them to you.

Save the changes to your form.

The Presentations Settings menu allows you to control how your feedback form is presented to your feedback partner.

Write a brief confirmation message that will be automatically sent to people when they respond to your survey.

Once you have decided on your Presentation Settings, hit Save.

Now, it’s your turn: Select and save your General Settings, and select your Presentation Settings.


Add a New Question Type

Add a multiple choice or checkbox question to your form.

In this extension, you will add one more question to your feedback form.

This time, you will choose one of two new question types: multiple choice and checkboxes.

In the main lesson, you worked with paragraph and short-answer formats.

These type-in formats allow your feedback partners to type in their own answers.

Think of a new question you can add to your feedback form.

Your question should either be a “yes or no” question, or a question that has a few possible answers, which you will write onto the form.

Take a few moments to think of a new question.

Then add it to your form.

Now select your question type.

If you have written a “yes or no” question, choose Multiple Choice.

Multiple Choice will also work for any question that requires your feedback partner to select just one answer.

If you chose multiple choice for your question type, add in the possible answers now.

“Checkboxes” are similar to multiple choice, but they allow your feedback partner to choose as many answers as they want.

Like multiple choice, checkboxes require you to list all the possible answers.

Now, it’s your turn: Add a new question to your form, choose either “Multiple Choice” or “Checkboxes,” and list your possible answers.
